When requesting a Certificate of Liability Insurance for an event, we must have the physical address and name of place where the event is being held. If that venue requires that they be shown as additional insured, our insurance company requires a signed contract stating that they must be shown as additional insured before the certificate can be issued by our Insurance company. If this is the case with your request, the signed contract must be included with the Certificate of Liability Insurance request. Additionally, a chapter can request a Certificate for their chapter which will show the Chapter as the Certificate holder which will cover them at most places. Please allow at least one week turnaround time for this process.
Certificates of Liability Insurance must be requested ONLY by submitting the online form or contacting Headquarters at (800) 247-2311. Certificates are valid from June 1-May 31.
